When a face sheet (i.e., patient demographics information) is needed, EMS is often able to obtain this at the time of hand-off. At times, information may not be available or complete. This process allows authorized EMS requesters to obtain the needed information from the OhioHealth Health Information Management team.
When may a Release of Information (ROI) Request be submitted?
When EMS is not able to obtain a completed face sheet at the time of hand-off for a patient they transported to or are transporting from an OhioHealth care site.
When may an EMS patient face sheet be released?
OhioHealth may provide a face sheet to authorized EMS requestors as allowed under HIPPA for payment and/or health care operations.
Who may request a face sheet?
The following individuals of an EMS, first responder, and medical transportation organization may request a face sheet for patients they transported to or are transporting from an OhioHealth care site:
- EMS personnel who directly provided patient care or transport
- EMS Supervisor or Coordinator
- EMS Medical Director
How do I request a face sheet?
Send a completed EMS Release of Information Request and a copy of the patient’s completed EMS medical record (e.g., patient care report) via secure email to release-of-information@ohiohealth.com.
If you do not have a secure email system the OhioHealth Secure Mail Center may be used to submit requests.
How do I receive a face sheet?
The face sheet will be sent to an authorized EMS requester by secure email to their organizational, government, or other verifiable email address or fax number as indicated in the completed EMS Release of Information Request.
Is there cost to receive a face sheet?
There is no cost to authorized EMS requesters.
